Insurance Information
Patients can apply for Medicaid and other public insurance programs onsite at Barrio Family Health Center or the Dr. Frank Bryant Health Center. Patients should bring proper identification and supporting documents.
Payment Information
CommuniCare Health Centers accepts:
More than 200 private health insurance plans
Public health insurance (Medicaid, Medicare, CHIP)
CommuniCare also serves individuals who are not covered by health insurance. These patients are charged for services based on a sliding fee scale based on the patient’s family income and family size. In accordance with Federal regulations, proof of income
must be provided for the patient to be eligible for any sliding fee discount on services. Examples of acceptable proof of income are payroll stub, Social Security statement, or child support documentation.
