Through its Public Housing Primary Care (PHPC) program, CommuniCare provides public housing and Section 8 residents with access to comprehensive primary healthcare, health education, and supportive services in order to improve their overall health and well-being. The PHPC program is funded through the U.S. Department of Health & Human Services, Bureau of Primary Health Care.
PHPC patients are referred into medical, dental, and behavioral health programs at Barrio Family Health Center and the Dr. Frank Bryant Health Center.
CommuniCare Health Centers provides comprehensive
primary health care services to all ages.
